Understanding Obesity: More Than Just Weight

What Is Obesity, Really? 

Obesity is often misunderstood as a simple result of eating too much or exercising too little. In reality, it is a chronic medical condition—one where excess body fat accumulates due to a complex interplay of genetics, hormones, metabolism, environment, and lifestyle. It is typically measured using the Body Mass Index (BMI), with a BMI of 25 and above considered overweight and 30 and above classified as obese (thresholds vary slightly for Asian populations, where health risks appear at lower BMI levels). 

The key insight from modern medicine is this: obesity is not a failure of willpower. It is a disease involving the body’s hunger and fullness signalling system—primarily hormones produced in the gut and brain that regulate appetite, energy storage, and metabolism. In many people living with obesity, these signals are dysregulated, creating a persistent drive to eat more and store fat, even when the person is making a genuine effort to eat less. 

How Obesity Affects the Body 

Excess fat, particularly around the abdomen and internal organs, does far more than change how we look—it actively disrupts how our organs function. Some of the major systems affected include: 

  • Metabolic system: Obesity is closely linked to insulin resistance and Type 2 diabetes. Excess fat impairs the body’s ability to use insulin effectively, leading to elevated blood sugar levels over time. 
  • Liver: One of the most under-recognised consequences of obesity is fatty liver disease (now termed MASLD—Metabolic dysfunction-Associated Steatotic Liver Disease). Fat accumulation in the liver can progress silently to inflammation, fibrosis, and, in advanced cases, cirrhosis. 
  • Cardiovascular system: Obesity raises the risk of high blood pressure, abnormal cholesterol levels, and heart disease, placing extra strain on the heart over years. 
  • Joints and mobility: Extra body weight increases mechanical stress on weight-bearing joints, contributing to osteoarthritis, particularly in the knees and hips. 
  • Hormonal and reproductive health: Conditions like PCOS (Polycystic Ovary Syndrome) and infertility are commonly linked with obesity due to hormonal imbalances. 
  • Digestive system: Obesity increases the risk of acid reflux (GERD), gallstones, and certain gastrointestinal cancers. 

The important takeaway is that obesity rarely stays contained to just “extra weight”. Left unaddressed, it becomes a root cause driving multiple chronic conditions simultaneously — which is why treating obesity effectively can create a ripple effect of improvement across the body. 

Available Treatment Options 

Obesity management has evolved significantly, moving beyond a one-size-fits-all approach. Treatment is typically personalised based on the severity of obesity, associated health conditions, and patient preference. 

1. Lifestyle Modification 

This remains the foundation of all obesity treatment — structured dietary changes, regular physical activity, behavioural therapy, and sleep optimisation. However, for many patients, lifestyle changes alone are not enough to overcome the underlying hormonal drivers of obesity, which is where medical therapy plays an important role. 

2. Medical Therapy — GLP-1-Based Medications 

One of the most significant advances in obesity treatment has been the development of GLP-1 receptor agonists — injectable medications that mimic a naturally occurring gut hormone called ‘Glucagon-Like Peptide-1’. 

Here’s how they help:

  • They slow gastric emptying, helping patients feel fuller for longer after meals. 
  • They act on appetite centres in the brain, reducing hunger and food cravings. 
  • They improve insulin sensitivity, which benefits blood sugar control. 
  • Used alongside lifestyle changes, they have shown meaningful and sustained weight reduction in clinical studies. 

Beyond weight loss itself, these medications have shown benefits for related conditions — including improvements in blood sugar control, fatty liver parameters, and cardiovascular risk factors — making them particularly relevant for patients whose obesity is accompanied by diabetes or fatty liver disease. 

These are prescription medications and require proper medical evaluation before starting, along with regular monitoring for effectiveness and side effects. They are not a stand-alone quick fix, but rather one part of a comprehensive, medically supervised treatment plan. 

3. Endoscopic and Surgical Options 

For patients with more advanced obesity or those who haven’t achieved adequate results with medical therapy, options such as endoscopic bariatric procedures or bariatric surgery may be considered, depending on individual evaluation. 

The Bigger Picture 

Obesity management today is not about a single intervention — it’s about a personalised, medically guided plan that treats obesity as the chronic condition it is. Because obesity acts as a root driver for so many other health issues, effective treatment can lead to improvements far beyond the number on the scale: better blood sugar control, a healthier liver, reduced joint strain, and lower cardiovascular risk.
