  • The Management sincerely requests you to take some time to fill out this form accurately.
  • This feedback helps the Management review and improve its services.
  • If you do not receive the form, please ask for it.
  • After completing the form, please post it in the designated box.
  • Your honest feedback is crucial for enhancing our services.

Medico-Legal Cases and Cashless Insurance Patients

  • For Medico-Legal Cases and Cashless Insurance Patients, investigation reports will be retained by the hospital.
  • You can request photocopies of these reports, which will be issued after approval from the Hospital Management.

Medicine Returns

  • Used or opened medicines will not be accepted for return.
  • Ensure all unused medicines are returned to the Pharmacy along with the original bill to obtain a refund.
