The term GOLDEN HOUR in trauma is used to underline the importance of urgent care which has to be given to critically injured patients soon after their injury .The Golden Hour refers to the first sixty minutes following a traumatic injury.
I feel everyone should know the importance of this critical Golden hour after a traumatic injury .
We being a trauma care centre would like to emphasize the importance of initial proper treatment immediately during this critical phase inorder to prevent mortality and also secondary injuries which happen due to delay in intervention during this period.
The first 60 minutes following trauma is a critical period for getting patients to a trauma center and has been called the “golden hour.”

Understanding the Golden Hour
The Golden Hour refers to the first sixty minutes following a traumatic injury. During this crucial window, the chances of survival are highest if the patient receives prompt and effective medical attention. First introduced in emergency medicine, the concept underscores that early assessment, stabilization, and treatment can dramatically improve outcomes for trauma patients. Even brief delays can worsen injuries, cause internal bleeding or organ failure, and, in severe cases, lead to death.
The physiology behind Golden hour
Trauma patients often suffer from massive haemorrhage, which can rapidly lead to hypovolaemic shock. Without timely intervention, the deadly cascade of hypoxia, acidosis, and coagulopathy — known as the lethal triad — sets in, making resuscitation increasingly difficult. Early control of bleeding, restoration of perfusion, and prevention of secondary injury are crucial during this critical period.
Role of prehospital care
The main role in prehospital care is to avoid unnecessary delays in shifting patient to trauma care centre .
They should also ensure patient is shifted with proper Cspine stabilzation on spine board
They should also ensure that proper oxygenation and ventilation is delivered to patient enroute to hospital
Early hemorrhage control with troniquets and compression bandage with immobilization of fractured limbs will also prevent hypotension in the early Golden hour .
Hospital response
Once the patient reaches the hospital we as emergency physician, start the early ressuscitaion with includes XABCDE

The Initial Assessment is also followed by activation of our trauma team for the definitive care required for the patient
The Golden Hour in trauma care is a foundational concept underscoring the urgency of early intervention in critically injured patients. Although trauma systems and technologies continue to evolve, the core principle remains unchanged: every second counts.
