A Story: The Pain That Came Between Ordinary Days<\/h2>\n\n\n\n


<\/strong>A 28 year old female ,married for a couple of years\u00a0 and a mother of two energetic children. Her eldest\u00a0 was six year old, and her youngest, baby\u00a0 was just \u00a011 months old<\/strong>\u2014still wobbling with each new step she took, giggling as she tried to keep up with her big brother.<\/p>\n\n\n\n

Life was busy and joyful and exhausting in all the usual ways. Between school drop-offs, late-night feedings, and juggling house-hold tasks, she rarely thought about herself. So when her period came late, she blamed it on stress, sleep deprivation, and the chaos of motherhood.<\/p>\n\n\n\n

Five Days Earlier: What she ignored<\/h2>\n\n\n\n

It started with a dull ache in her lower abdomen\u2014slightly stronger than usual, but nothing alarming. assuming her cycle was just being unpredictable again, the way it often was since having her second child .The bleeding continued through the day, not heavy, not light\u2014just enough to convince her it was menstruation.<\/p>\n\n\n\n

The Morning It Became Impossible to Ignore<\/h3>\n\n\n\n

On the 5th<\/sup> day of morning, the abdominal pain came in waves. It wasn\u2019t normal, but she brushed it off initially and within an hour she experienced a sudden, stabbing pain shot through her left lower abdomen.<\/p>\n\n\n\n

Day 5: When She Finally Sought Help<\/h3>\n\n\n\n

As the pain grew so intense that she could barely finish house hold works. Her abdomen throbbed, and waves of nausea came and went.<\/p>\n\n\n\n

Visit to Nearby Clinic (Initial Management)<\/h3>\n\n\n\n

Symptomatic pain management treatment given and kept her  for observation ,after few hours patient BP fallen down , Refer to higher centre.<\/p>\n\n\n\n

Arrival at Emergency Department<\/h2>\n\n\n\n

At the Emergency Room :<\/strong><\/h3>\n\n\n\n
    \n
  • Patient looks pale, drowsy , and in severe diffused abdomen pain.<\/li>\n\n\n\n
  • Vitals: BP 70\/30 mmHg, Pulse 160\/min ,RR 30\/min, GCS – E3 V4 M6 ,GRBS – 531mg\/dl<\/li>\n<\/ul>\n\n\n\n

    Examination :<\/h3>\n\n\n\n
      \n
    • Abdomen: Marked diffuse tenderness present with more prominent in the left iliac fossa with guarding and distension of abdomen.<\/li>\n\n\n\n
    • CNS examination : Drowsy , obeys verbal commands.
      <\/li>\n<\/ul>\n\n\n\n

      Investigation at Emergency Room :<\/h3>\n\n\n\n
        \n
      • Urine pregnancy test:<\/strong> Strongly positive<\/li>\n\n\n\n
      • ABG<\/strong> : pH – 7.15 , pCO2 – 42.7 , HCO3 – 13.4 , Hb – 6.3<\/strong> , Glucose – 531 mg\/dl<\/li>\n\n\n\n
      • Serum \u03b2-hCG:<\/strong> 4786 IU\/L<\/strong><\/li>\n\n\n\n
      • USG abdomen and pelvis :\u00a0<\/strong><\/li>\n\n\n\n
      • Gross ascites with internal echoes\u00a0<\/li>\n\n\n\n
      • \u00a0Ill defined heterogenous lesion in left adnexa and pelvis – likely hematoma.<\/li>\n\n\n\n
      • Small heterogeneous left adnexa cyst\u00a0<\/li>\n<\/ul>\n\n\n\n

        Diagnosis<\/h2>\n\n\n\n

        The findings were consistent with a Left tubal ectopic pregnancy<\/strong>, with rupture.<\/strong><\/p>\n\n\n\n

        She asked softly, \u201cSo\u2026 does this mean I\u2019m losing the pregnancy?\u201d Then we reassured her that it wasn\u2019t her fault and that they had caught it before it became life-threatening . We will be removing the affected left fallopian tube through tiny keyhole incision under anaesthesia , and you can still conceive normally with your healthy remaining tube.<\/p>\n\n\n\n

        Management<\/h2>\n\n\n\n

        Emergency Care<\/h3>\n\n\n\n
          \n
        • IV fluids, oxygen given , Blood Transfusion<\/li>\n\n\n\n
        • Pain management

          <\/li>\n<\/ul>\n\n\n\n

          Definitive Management<\/h4>\n\n\n\n
            \n
          • Due to hemodynamic instability, she was immediately taken for laparoscopic left salpingectomy<\/strong>.<\/li>\n\n\n\n
          • Around 1200mL of hemoperitoneum<\/strong> noted.
            <\/li>\n<\/ul>\n\n\n\n

            Outcome <\/h4>\n\n\n\n

            She recovered well post-operatively . She asked about her future fertility, and the gynecologist explained that many women conceive normally with one fallopian tube. She was referred for follow-up and emotional support.<\/p>\n\n\n\n

            She left the hospital two days later, grateful for the care she received and relieved that she had sought help when she did. <\/p>\n\n\n\n

            Discharged  with advice on : Early scanning in future pregnancies , Recognizing warning symptoms<\/p>\n\n\n\n

            Pitfalls<\/h4>\n\n\n\n

            1. Dismissing spotting as a \u201cnormal early period\u201d.<\/p>\n\n\n\n

            2. Attributing abdominal pain to stress or non- gynecologic causes.<\/p>\n\n\n\n

            3. Underestimating symptoms \u2013 misdiagnosis or delayed diagnosis.<\/p>\n\n\n\n

            4. Over-reliance on the absence of severe bleeding<\/p>\n\n\n\n

            5. Assuming a lack of extensive risk factors lowers suspicion<\/p>\n\n\n\n

            6. Potential delay if the ultrasound was not interpreted urgently<\/p>\n\n\n\n

            7. Missing early signs of internal bleeding<\/p>\n\n\n\n

            8. Not exploring and addressing emotional impact<\/p>\n\n\n\n
